The Choice Plans Rx 4 Tier Drug program is an information and service program that provides enhanced pharmacy services, offers better control and lower prescription costs to members. The 4 different tiers allow you to choose the exact plan which will offer you the largest savings on your prescription drug expenses. With over 50,000 participating pharmacies, you can save money on your prescriptions no matter where you are. Plans start at $25.99 per month for individuals, and $34.99 per month for families.

Tier 1: Consists of Generic Drugs that can be purchased for $10.00 or less.

Tier 2: Consists of Brand Name and Generic Drugs that can be purchased for $20.00 or less.

Tier 3: Consists of Brand Name and select Generic Drugs that can be purchased for $40.00 or less.

Tier 4: Consists of Brand Name and select Generic Drugs that can be purchased for a special discounted price.
